Sports Forms and Directions
All students participating in sports at Evangel must complete the Student Sports Physical form and return it to the coach along with the sports fee. Homeschoolers may participate in the sports program at Evangel if the teams are in need of additional players. Homeschoolers must complete both the Student Sports Physical form and the Homeschoolers Sports Application.
