The following forms must be completed prior to Step 1 of the admissins process:
Admission Application
Pastoral Reference (for all students)
7th through 12th Grade Student Questionnaire
Teacher Reference
Once the student has been notified of acceptance (Step 4) for admission to the school, the following forms must be completed to finish the admission process.
Statement of Coopoeration and Permission
Emergency Care Information
School Records Request Form
Prince William County School Entrance Health Form
The following form must be completed for reenrollment each new school year. Reenrollment for current ECS students begins February 1st.
